The instructions for Betsy Lehman’s breast cancer treatment were ambiguous: “Cyclophosphamide 4 grams per square meter of body surface area over four days.” A doctor at the Dana-Farber Cancer Institute in Boston read the dosage as 4 grams per day, rather than the intended 4 grams spread over four days.
